Submit a request
Before filling out the form, select the desired submission type:
- A request for public information is submitted if you want to receive information or copies of documents held by the Accounting Chamber.
- An electronic appeal is submitted if you want to make a proposal, submit an application or complaint, report a problem or raise an issue that requires consideration.
In matters of access to public information, the Accounting Chamber is guided by the following regulatory legal acts:
- Law of Ukraine «On Access to Public Information»;
- Law of Ukraine «On Information».
According to the Law of Ukraine «On Access to Public Information» public information is information reflected and documented by any means and on any media that was received or created in the process of performing by subjects of public authority their duties stipulated by current legislation, or which is in the possession of subjects of public authority.
The provision of public information by the Accounting Chamber is carried out in response to a request for information.
A request for information is a request from a person to the information manager to provide public information in his possession.
The requester has the right to contact the information manager with a request for information, regardless of whether this information concerns him personally or not, without explaining the reason for submitting the request.
A request for information can be individual or collective. Requests can be submitted orally, in writing or in another form (by mail, telephone, e-mail) at the requester's choice.
A written request is submitted in any form or using the information request form. (Download the form in the format PDF\DOCX).
In accordance with Part Five of Article 19 of the Law of Ukraine "On Access to Public Information" It is established that a request for information must contain:
1) the name (title) of the requester, postal address or e-mail address, as well as the number of the means of communication, if any;
2) a general description of the information or the type, title, details or content of the document for which the request is made, if the requester knows this;
3) signature and date, provided that the request is submitted in writing.
Fee for providing information
Information upon request is provided free of charge.
If satisfying a request for information involves making copies of documents exceeding 10 pages (starting from the eleventh page),the requester is obliged to reimburse the actual costs of copying and printing.
When providing a person with information about themselves and information of public interest, no fee is charged for copying and printing.
Appealing decisions, actions or inaction of information managers
Decisions, actions or inaction of information managers may be appealed to the head of the manager or to the court.
Appealing decisions, actions or inaction of information managers to the court is carried out in accordance with the Code of Administrative Procedure of Ukraine.

A special structural unit that organizes access to public information in accordance with the established procedure, is responsible for considering, processing, accounting, systematizing, analyzing and providing responses to requests for information received by the Accounting Chamber, and also provides consultations during the processing of such requests is designated The Department of Information activities and communications.

The application is made in any form. The attached file must be in one of the following formats: pdf, docx, jpeg, png.
- In matters of access to public information, the Accounting Chamber is guided by the requirements of the Law of Ukraine “On Citizens’ Appeals”.
The appeal must indicate the citizen’s last name, first name, patronymic, and place of residence. The written appeal must be signed by the applicant(s) with the date. The electronic appeal must also indicate the email address to which the applicant can be replied, or information about other means of communication with him/her. The use of a qualified electronic signature when sending an electronic appeal is not required.
A written appeal without indicating the place of residence, not signed by the author(s),as well as one from which it is impossible to establish authorship, is recognized as anonymous and is not subject to consideration.
